CentMed is open Monday to Friday from 9:00 AM to 5:00 PM and Saturday from 9:00 AM to 1:00 PM. We’re closed on Sundays. Walk-ins are welcome during these hours, or call (773) 545-2525 to schedule an appointment.

For general visits, bring a photo ID and insurance card (if applicable). For immigration exams, bring Form I-693 (downloadable here), vaccination records, and ID. No fasting is required unless specified. New patients can fill out registration forms online here.
