Medical Exams
Required for Green Card (Adjustment of Status) and various immigrant/non-immigrant visas.
The immigration medical exam determines if an individual has health conditions that would make them generally inadmissible to the United States on medical grounds. Performed at our Chicago clinic by USCIS-designated Civil Surgeons.
Important COVID-19 Mandate: Effective October 1, 2021, all applicants for lawful permanent residence must be fully vaccinated against COVID-19. Proof of vaccination is mandatory before a Civil Surgeon can sign your I-693 report.
The exam includes a full physical (disrobing required), mental status evaluation, and screenings for Class A conditions deemed a threat to public safety:
It also covers drug addiction and psychiatric disorders. If a treatable condition is found, our doctors will recommend a course of treatment.
Conditions not deemed a public safety threat are Class B. Applicants with Class A conditions may apply for medical waivers (Form I-601), specifically if they are the spouse or unmarried child of a U.S. citizen or permanent resident.
Prior to your visit, download the Official USCIS I-693 Form. After the exam, the Surgeon will provide a sealed envelope. Do not open it; it must be submitted sealed to USCIS.
